A person wanting to receive Holy Communion at home, due to a medical condition that is preventing Mass attendance, should first call theSJCC Parish Office 904-268-5422. Information will be gathered; their needs will be assessed for what SJCC can provide and next steps will be taken.
The request for Holy Communion at home is the initial request, but there are other things to consider: parish registration, geographical area, the need to see a priest for counsel, Confession, Anointing, and determining if there are other needs that can be handled through SJCC or directed to the appropriate agency. This initial process needs to be handled through the SJCC Office.
When the person is ready to receive Holy Communion at home through the Homebound Ministry an email will be sent to the Homebound Ministry contact person (Jackie West) and Deacon Bob Gardner. The email will give a brief description of the person's situation, their name, address and the contact person's name and phone number.
Jackie West visits the person the following weekend, has a Communion Service and gets to know the person. This may take a visit or two, depending on the situation. Sometimes, especially after surgery, two or three visits are all that is needed. When ready, they will be transitioned into the Homebound Ministry rotation schedule with the other EMHC.
